WorkSafe says it is continuing to investigate the death of a worker at Hofmann Engineering in Bassendean on Monday afternoon.

Acting WorkSafe commissioner Jennifer Low said any work-related death was a tragedy and relayed sincere condolences to the worker’s family and workmates.

“It’s clearly a concern to WorkSafe that a second death has occurred at this workplace,” Ms Low said.

She said the mature age apprentice in his 40s was working near a steel object when it fell onto him, resulting in fatal crush injuries.

WorkSafe inspectors attended the site shortly after receiving notification and will continue to examine the incident scene and the circumstances that led to the incident.

WorkSafe also confirmed that Hofmann Engineering was prosecuted and fined $567,000 in November 2024 over a previous worker death at the same workplace in 2019.

“WorkSafe will continue to conduct a thorough investigation to identify what caused this incident and whether any potential offences have been committed.

“As always, once the investigation has concluded we will share any learnings with the industry as part of our commitment to supporting safe and healthy work practices in all workplaces.”
